We have an exciting Locum Social Work position within the Leaving Care service, based in Sefton, Merseyside. You'll work with young people aged 16-18, helping them transition from care to independent adulthood, offering critical support as they take this important step.

Key Responsibilities:
- Provide direct support and guidance to young people, ensuring their transition to adulthood is smooth and successful
- Build meaningful relationships with young people and their families, ensuring their involvement in assessment, planning, and decision-making processes
- Manage a varied caseload, ensuring every young person receives the tailored support they need
- Work collaboratively with partner agencies, advocating for the young people and securing essential services
- Tailor your support based on best evidence and address key risks in each young person's life
Ideally you will have:
- 1-3 years' experience in a similar role
- Diploma/Degree in Social Work and registration with Social Work England
- Experience with Pathway Planning for looked after children and care leavers
- Strong partnership experience, particularly in housing and joint homelessness assessments, would be highly desirable
