In this role, you will provide physical care to the residents to meet their daily needs and enable them to function at the highest possible level. While this care is provided under the leadership of a professional nurse, the Care Partner is the resident's closest advocate in the household. You will promote the residents' psychosocial well-being through meaningful relationship building, and you will work with the household team to meet other social service needs of the resident and of the household.Who you are:You are an empathetic team-oriented person, who truly represents our values: Commitment, Compassion, Holistic Care, and Innovation.
