An exciting job role has recently become available at the Sefton Council. They are looking for a new member for their Adult Learning Disability & Autism Team. Prepare statements and represent clients in court, working within legislative frameworks. Make complex decisions using professional judgment, involving service users and needs-led assessments. Assess risk and need in complex cases, conduct safeguarding investigations, and maintain updated care plans. Develop care plans with other agencies, ensuring they reflect service user wishes and lead to personalized care. Negotiate care arrangements with individuals, balancing choice with budget constraints. Collaborate across multi-agency teams and undertake direct work with adults and their families.
