Qualified Social Worker | Hospital Team |London Borough of Bromley Qualified Social Worker required to join a statutory Hospital Team Office base: Bromley - Civic Centre To be considered, you must have a social work qualification and full SWE registration as well as over 3 years post qualified experience gained in an Adults Services setting £32phThis is an experienced practitioner role, expected to undertake cases independently and demonstrating an ability to adapt the approach from first principles to deal with more complex or unusual case, assessing and managing risks appropriately. The role is for Hospital Social Worker. We are looking for someone with hospital Social Work experience, able to assess and facilitate safe discharges in a timely manner. This is a fast-paced team working collaboratively with health partners, housing, and other stakeholders to facilitate hospital discharges from acute settings. The management and team are very supportive. The team employ hybrid work with some days working from home depending on the needs of the service. The candidate should be able to work with children (see the adult, see the children) and young people, families, carers, and communities to help them express their needs and make informed decisions and contribute to service planning.
