Qualified Social Worker | Adults Assessment |London Borough of Bromley Qualified Social Worker required to join a statutory discharge from neuro-rehab units Office base: Bromley - Civic Centre To be considered, you must have a social work qualification and full SWE registration as well as over 3 years post qualified experience gained in an Adults Services setting £32phLocum SW required to support discharge from neuro-rehab units Complete Care Act Assessment and Mental Capacity Assessments Attend daily conference call in the mornings at 08:30 Attend once a week multi-disciplinary team meeting on Tuesdays Support the rehab ward team with any client who requires social service input Complete joint home visit with rehab therapists when needed Liaise with other multi agencies Complete Safeguarding/ Dols Complete Placement, Extra Care Housing (ECH) Promote direct payment Present cases to Practice Review Group Knowledge regarding community resources Attend face to face meetingPlease apply to be considered If you are interested in discussing this position in more detail please call (phone number removed) and ask for Anton Mostert or email (url removed) Caritas Recruitment are a specialist recruitment agency for Qualified Social Workers at all levels.
