Job Summary

We are seeking a skilled RN Case Manager to join our team at UF Health Shands. As a key member of our healthcare team, you will be responsible for assessing, planning, implementing, and monitoring comprehensive care plans for patients. Your expertise will be essential in collaborating with other healthcare professionals to ensure seamless transitions and effective care.

Key Responsibilities

• Assess patient needs and develop individualized care plans
• Collaborate with healthcare teams to ensure effective care coordination
• Monitor patient progress and make adjustments to care plans as needed
• Communicate with patients and families to ensure understanding and satisfaction

Requirements

• Valid RN license in the State of Florida
• 2 years recent RN experience in a clinical capacity
• Prior Case Management experience preferred
• BSN or MSN degree preferred
