In this role, you will play a vital part in helping individuals with multiple and complex needs prevent a return to rough sleeping. You will provide early intervention, ongoing support, and guidance following discharge from the hospital. What you'll be doing: Offering guidance and developing support plans to ensure clients sustain their quality of life Engaging with various support agencies to promote personal independence and well-being Acting as the main point of contact for casework within the accommodation, coordinating solutions for drug and alcohol, domestic violence, and mental health issuesWhat we're looking for: Demonstrable experience in case planning and complex case management Sensitivity to the diverse needs of individuals from a range of backgrounds Strong communication skills, both written and verbal Knowledge of local social and health care services and the ability to navigate systems and pathwaysQualifications and skills: Experience working independently and providing high-quality care Proficiency in Microsoft Word, Excel, and Outlook Understanding of the wider issues impacting homelessness and the challenges faced by individuals with complex needsJoin our client's passionate team and contribute to creating positive change in our community.
