A bit about the role

As a Team Leader, your role involves coordinating case management for our young person. You'll take the lead in delivering our therapeutic approach driven by love and commitment. About the service

As a Team Leader in our Peterborough service, you'll be supporting the transition our of young person into their own forever home!

They've been living in their current setting in Hampshire for some time and therefore our mission is to bring them home, where they'll be close to family and receive the stability, consistency, and appropriate care they need.

Our approach is therapeutic, driven by love and a commitment to helping the young person thrive. We aim to integrate them into the community as much as possible by supporting and encouraging independence as they transition into adulthood.

Working closely with partner agencies including health, education, and social care partners, as well as the young person's family, we'll provide high-quality support using a trauma-informed approach. These relationships will enable our young person to do all of the things they enjoy.
